Report
of birth in Japanese
When you give birth during your stay
in Japan, you are requested to report it to the city office within
14 days.
Ask the physician of the hospital or
clinic where you gave birth to fill in the notification form
for a Report of Birth and submit it to the city office after
you fill it out.
This report may also be necessary for
your baby to obtain citizenship of your home country in the future.
When you report, please bring your Maternal and Child Health
handbook (Boshi-Kenko-Techo) with you. If your baby will stay
in Japan more than 60 days after birth, Non-Japanese Registration
is also required. When the parents are non-Japanese and both
the mother and father have different nationalities, the report
must be submitted to the authorities of both parent's home countries.

Report
of death
Regardless of the nationality of the
deceased, the bereaved family must report the death to the city
office within 7 days after the death. The report form is attached
to the certificate of death prepared by a doctor. If the deceased
died in an accident or the cause of death is questioned, the
police will become involved in the procedures.
When you wish to cremate or bury a body
in Japan, you must obtain permission for cremation or burial
from the City Office. Burial is not ordinarily permitted in Japan
for sanitary reasons. If you want to bury the body in your home
country, contact your own country's embassy or a consulate in
Japan. (See page -- for embassy or consulate numbers)
